Highland Rivers Provides Community Services

We are here and expanding our services to be more mobile in Gilmer County and Fannin County!!!

Highland Rivers Community Service Board (HR CSB) continues to provide:

Community Support Team/CST: This team is made up of 3 individuals made up of an RN Team Lead, Bachelor’s Level Paraprofessional, and a Certified Peer Specialist.
CST is an in home/community service that provides restorative/recovery-focused intervention to assist consumers with:
1. Gaining access to necessary services;
2. Managing (including teaching skills to self-manage) their psychiatric and, if
indicated, co-occurring addictive and physical diseases;
3. Developing optimal independent community living skills;
4. Achieving a stable living arrangement (independently or supported); and
5. Setting and attaining consumer-defined recovery goals.

Peer Support Program: The Gilmer Peer Support Program will be a satellite program of the Pickens OP clinic. This program will continue to offer services Tuesday, Wednesday and Friday from our Cherry Log location. Peer Support provides structured activities within a peer support center that promote socialization, recovery, wellness, self-advocacy, development of natural supports, and maintenance of community living skills. Activities are provided between and among individuals who have common issues and needs, are consumer motivated, initiated and/or managed, and assist individuals in living as independently as possible.

Community Support Services (CSS): CSS is in-home/community based individual skill building and resource linkage

Veterans Mental Health Court/Drug Court/Mental Health Court Collaboration: HR CSB will continue to collaborate and coordinate services with each of these to meet the needs of the consumers served through these courts.

Clinic-based Outpatient Services will continue to be available to Gilmer/Fannin consumers at the Pickens OP Clinic in Jasper, GA providing: behavioral health assessment, individual therapy, group therapy, diagnostic assessment, psychiatric treatment, and nursing services.
